Please note the following important information:
- Please read the appointment letter that will be emailed or mailed to you prior to your consultation.
- Please respond promptly to the SMS Appointment Confirmation message we send you with a Y to confirm your appointment (call or email us if you need to change your appointment).
- Ensure you have completed the Patient Registration Form and Patient Health History Form.
- Bring your current Medicare and concession (if applicable) cards with you so we can process your medicare rebate for you.
Please allow approximately 2 hours for your appointment. We try to run to schedule but emergencies can occur, and we appreciate your patience if your appointment is delayed.
